EXTENDED REGISTRATION FOR KATIPUNAN, TAMPILISAN, and SIOCON CAMPUSES
(for Katipunan campus December 19-26, 2023)
1. The application may close anytime as soon as the absoptive capacity of each campus is fully met.
2. The applicants shall take the admission test in the campus where the application is lodged.
3. The qualified applicants shall enroll in the campus where the examination was taken and the results obtained.
4. For qualified applicants who desire to transfer to other campus, they shall be considered second priority subject to the availability of slots of the preferred program.
For SHS Graduate:
-Photocopy and Original copy of Report Card
-Phocopy and Original copy of PSA Birth Certificate
For SHS Graduating:
-Certificate of Enrollment as G12 student for S.Y. 2023-2024 signed by the School Principal or Guidance Counselor
-Phocopy and Original copy of PSA Birth Certificate
For TRANSFEREES:
-Photocopy and Original copy of TOR OR Informative Copy (Higher year) OR Certificate of Enrollment signed by the School Registrar (1st year)
-Phocopy and Original copy of PSA Birth Certificate
For ALS PASSERS:
-Certificate of Rating or Certificate of Eligibility
-Phocopy and Original copy of PSA Birth Certificate
